The Milano Marathon runs point to point through the city center, and the course is genuinely flat. You'll pass the Duomo, thread through piazzas, and cover stretches of wide urban boulevard where you can find your rhythm early and hold it. The crowds are engaged and the atmosphere is warm. It's a course where you can actually execute a race plan, which is rarer than it sounds.

For PR chasers, this is a real option. The elevation change is minimal, open registration means you can sign up without a lottery or qualifying standard, and the April date usually brings cool mornings that suit fast running. A charity bib is also available if you want the flexibility of a fundraising spot. If you're coming from abroad, Milan has good airport connections and the city center has everything you need within walking distance of the start. Get your legs moving the day before and don't overthink it.

The course is essentially flat, running point to point across Milan with minimal elevation change throughout. The highest and lowest points differ by under 60 feet, and the cumulative gain is modest enough that it plays no real role in pacing. There are no meaningful climbs to prepare for and no long descents to manage. The main variables are road surface, turns through city streets, and any wind exposure on open stretches. April in Milan is cool but not cold. The start can feel brisk, so a throwaway layer in the corral is a good idea. By the later miles you'll have warmed into it, and on a typical day conditions stay comfortable enough to race without managing heat. Wind through the city streets is the bigger variable, not temperature. Generally a favorable window for running well.
